01

Gene therapy technology continues to make breakthroughs, and precision medicine accelerates into a new stage

Gene therapy is driving the shift in disease treatment models from traditional symptom control to etiological intervention. With the maturity of technologies such as gene editing, delivery systems, and in vitro cell engineering, gene therapy has been gradually applied in multiple fields including hereditary eye diseases, blood system diseases, neurological diseases, and tumors, becoming an important direction of precision medicine.

Currently, gene therapy mainly forms four core technical paths: gene replacement, gene supplementation, gene expression regulation, and gene editing. Among them, gene replacement and gene supplementation restore or enhance cellular functions by introducing functional genes; gene expression regulation intervenes in the progression of diseases by regulating the expression level of specific genes; gene editing uses tools such as CRISPR-Cas to precisely modify target genes, providing new treatment options for genetic diseases and some chronic diseases. At the same time, virus vectors and non-viral vectors technologies continue to develop, optimizing the gene therapy delivery system. Virus vectors, with high delivery efficiency and a mature clinical application foundation, remain an important technical route for gene therapy; non-viral delivery systems, represented by lipid nanoparticles (LNP), have become an important supplementary direction due to their large payload capacity, low immunogenicity, and relatively flexible production process.

02

Global gene therapy indications continue to expand, and clinical applications extend

As the global gene therapy R&D pipeline continues to grow, clinical applications are expanding from rare genetic diseases with relatively clear pathogenic mechanisms to areas such as tumors, neurological diseases, and some cardiovascular and metabolic diseases.

Currently, hereditary eye diseases, hemophilia, and some rare diseases have become important areas for gene therapy product development and commercial application due to relatively clear patient classification, high disease burden, and long-term treatment needs. Significant progress has been made in gene therapy for hereditary blood system diseases such as transfusion-dependent beta-thalassemia and hemophilia, providing a new path for patients to transition from long-term supportive treatment to potentially long-term beneficial treatment.

In the field of central nervous system diseases, gene therapy provides new technical pathways for breaking through the blood-brain barrier and achieving targeted delivery of therapeutic genes through intrathecal administration, brain-targeted injection, and engineered AAV vectors, but further verification of long-term efficacy and safety is still needed. In the field of cardiovascular and metabolic diseases, gene therapy is gradually expanding to hereditary, refractory, and high-risk patient groups. In the tumor field, gene-modified cell therapies such as CAR-T have been commercially applied in blood system malignancies, and future expansion to solid tumors still requires overcoming key challenges such as target selection and safety control. With continued research on delivery efficiency, tissue targeting, and long-term safety, the clinical application boundaries of gene therapy are expected to be further expanded, providing new treatment options for more disease areas.

03

The commercialization pattern of gene therapy evolves rapidly, and the value realization path of innovative therapies expands

As global gene therapy products are successively approved for market release, the industry is moving from early technical validation to commercial application stages. Whether innovative therapies can achieve commercialization depends not only on technical breakthroughs and clinical efficacy but also on multiple factors such as production costs, payment capabilities, and long-term treatment value.

From the perspective of commercial development paths, disease areas with high disease burden, high unmet clinical needs, and high long-term treatment costs are more likely to see commercial breakthroughs first. Gene therapy's potential value of achieving long-term clinical benefits through one-time or infrequent interventions provides a new option for traditional disease management models, extending economic evaluation from single-treatment costs to long-term patient treatment and disease management costs. At the same time, there is a time mismatch between the high cost of single treatments and long-term benefits of gene therapy, posing new challenges to the traditional payment system. Innovative payment models such as commercial health insurance, installment payments, and value-based payments are becoming important exploration directions to reduce patients' short-term payment burdens and improve access to high-value innovative therapies.

04

The global regulatory framework continues to improve, and international cooperation promotes the coordinated development of the gene therapy industry

As the global gene therapy industry accelerates toward clinical translation and commercial application stages, continuous improvement of regulatory policies and deepening international cooperation are becoming important forces driving industry development.

In recent years, the global gene therapy-related regulatory framework has been gradually improved. Relevant technical guidance principles and review and approval mechanisms for pharmaceutical research, clinical trials, product quality control, and long-term follow-up have been refined, helping to reduce uncertainty in the development and registration of innovative products and supporting the clinical translation and commercial development of gene therapy products. At the same time, cross-border technology licensing, strategic cooperation, and joint R&D activities in the global gene therapy field are continuously active. International cooperation models are gradually evolving from single-product licensing to technology platform cooperation, global clinical development, and diversified commercial cooperation. Large pharmaceutical companies and innovative biotech companies are accelerating innovative technology R&D and clinical translation through resource integration and complementary advantages, promoting a more integrated innovation cooperation network for the global gene therapy industry.

05

Platform-based R&D and large-scale applications are advancing together, enabling the gene therapy industry to move towards high-quality development

The global gene therapy industry is moving from early technical exploration and clinical concept validation stages to a new stage of platform-based R&D, multi-pipeline layout, and large-scale industrial application. In the future, the R&D model will shift from developing single candidate drugs to building platform technologies, expanding indication matrices, and developing multiple pipelines; clinical applications will extend from areas with relatively clear pathogenic mechanisms such as rare genetic diseases and eye diseases to a wider range of diseases; industrial competition will shift from single-tech advantages to a comprehensive competition in clinical value verification, production process stability, quality control, and commercialization capabilities.

The Blue Book systematically reviews core technology evolution, clinical applications, global and Chinese market patterns, regulatory policies, and future development trends, providing industry references and trend analysis for biopharmaceutical companies, research institutions, investment firms, and all stakeholders in the industrial chain. Together, they promote the gene therapy industry to move toward standardization, scale, and sustainable development under the synergistic drive of technological innovation, clinical value verification, quality system improvement, and commercialization capability enhancement.
